New Appointments:

Dr. Gochberg, Dr. DuBois, Dr. Crosby, and Katie Southworth are accepting new patients!

Dr. Kaltunas is currently closed to new patients. 

You will be emailed a link from Elation, our EHR, to complete medical and consent forms prior to your visit. Please arrive 15 minutes early to your first appointment. Filling out forms prior to your visit ensures timely care. 

Follow-up visits: 

It is highly recommended that you also schedule a follow-up visit 2-3 weeks following your initial visit to ensure you can get on the schedule in time.
